Karnim:

It consists of 6,8 and 10 Gingerols which cause inhibition of Na+ and K+ pump. Reduced K+ conductance cause membranes depolarisation and reflux of Ca++ ions through voltage sensitive Ca++ channels which ultimately stimulates insulin secretion by beta cells of pancreas. Thus Ginger increases Blood insulin level and decreases lipid level. Ginger acts as Digestive and increases Gastro-intestinal motility.

Tulsi (Ocimum sanctum) promotes uptake of Glucose by peripheral tissues by decreasing peripheral resistance to insulin. Tulsi has antistress activity due to which it effectively takes care of stress precipitated aggravation hyperglycemia. Pharmacology:

Karela (M. charantia)- Karela popularly known by scientists as plant insulin exerts it antidiabetic property by means of both pancreatic and extra pancreatic effects. The antidiabetic property of karela is due to two constituents of its fruit; a 17 amino acid polypeptide and charantin which is a mixture of two steroid glycoside. The ‘insulinornimetic’ property of karela is due to the 17 amino acid polypeptide which structurally resembles to a part of bovine insulin. The charantin bears a hypolglycemic property.

Neem (A. indica)- The active principle of neem, nimbidin is hypoglycemic in activity; it is also shown to stimulate the insulin secretion by activating the beta cells of islets of Langerhans in pancreas. Besides this primary activity, neem also corrects the immune system of body which usually deteriorates in diabetic condition.

Tulsi ( O. sanctum)- Tulsi promotes uptake of peripheral glucose most likely either by decreasing peripheral resistance to insulin or by any other action leading to promotion of insulin – receptor interaction. Tulsi is also known to possess antistress activity and thereby takes care of stress induced glucose intolerance and hyperglycemia. Secondly tulsi is shown to potentiate Action of exogenous insulin.

Kutki (P. karroa)- It acts as an immunomodulator by means of its irroid glycoside fraction which potentates the immune system. It is known to have a hepato-protective activity by means of restoring glycogen level. Sulfonyl urea like glibenclamide are known to precipitate cholestasis during diabetic therapy. Adjuvant therapy with Karnim which contain P. kurroa thus provide protection and prevent the toxic effects of allopathic antidiabetic drugs on liver.

Sounth (Z. officinale)- It has got a stimulatory effect on pancreas. This action is attributed to 6,8 &10 gingerols, which are assumed to cause inhibition of NA+ / K+ pump. This action depolarizes the cell membrane resulting in influx of Ca++ ions though voltage sensitive Ca++ channels which ultimately stimulates insulin secretion by beta cells of pancreas. Secondly it lowers the blood lipid level which is a common feature in diabetes due to excessive consumption of fats, which may onset atheroscleroses.

Purified Guggul (C. mukul)- One of the major complication in diabetes is elevated levels of very low-density lipoprotein (VLDL) in blood circulation. The VLDL is converted to more cholesterol rich LDL which causes onset of atherosclerosis in diabetic patients. The situation is more complicated in obese patients in whom the triglyceride levels are quite eleveted. C. mukul is a well known anti-cholesteremic and antihyperlipidemic agent which effectively prevents capillary thickening and subsequent pathological consequences in diabetes. This effect of C. mukul coupled with its fibrinolytic activity, also prevents retinopathy, neuropathy and gangerene which is secondary to atherosclerosis and precipitated when already narrowed blood vessels are blocked by blood clots (fibrin).

Side Effects:

Karnim does not produce side effects such as nausea, vomiting, diarrhoea, rash and giddiness.

Special Precaution:

Strict dietary control and a regular exercise regimen must accompany Karnim therapy. Regular monitoring of blood sugar level is necessary.

Dosage:

Recently diagnosed patient, not taking oral hypoglycemic drugs- 1 to 2 Capsules three times a day, half an hour before meals.

Patients using Oral Hypoglycemic drugs and /or Insulin Injections- One capsule three times a day before meals.
